Terms & Conditions for Release of Liability
I acknowledge that participation in the above trip/event involves risk to the individual listed above as the participant and may result in various types of injury including, but not limited to, the following: sickness, bodily injury, death, emotional injury, personal injury, property damage and financial damage.
In consideration of the Participant being accepted for participation in activities and trips sponsored by Campus Outreach Lexington / Tates Creek Presbyterian Church, Inc (“CO / TCPCA”), the undersigned hereby waive and releases Tates Creek Presbyterian Church, Inc., its employees, agents, trustees, officers and members, and any affiliate or subsidiary of Tates Creek Presbyterian Church, Inc. and its employees, agents, officers, members, and managers, from any liability for personal injury to the undersigned or loss or damage to undersigned’s property that may result from or arise out of the undersigned’s participation, involvement, travel to and from, travel during the event, volunteer work, food preparation, playing or participating in activities, housing arrangements, including any personal injury, property loss or damage that may be caused by or arise from the negligence of any other participant, non-participant, or any employee, agent, trustee, managers, officers or members of Tates Creek Presbyterian Church, Inc. or its affiliates or subsidiaries committed during the course of said employees, CO staff, CO director, TCPC Administrators, TCPC staff, TCPC Pastors, trustees, officers, or member’s participation or involvement in the aforesaid activities with participant during the event/trip.
It is acknowledged and agreed that participation in any CO / TCPCA sponsored activity or trip is conditioned upon Participant complying with all rules and directions of persons in authority. In the event that the Participant does not comply with said rules or directions, CO / TCPCA reserves the right to require Participant to return home at her or his own cost, and may hold Participant responsible for any property damage or personal injury resulting from the Participant’s misconduct in not complying with all rules and directions of persons in authority.
If the participant is under the age of 18 then a parent or guardian’s signature is required.
This authorization, consent, and waiver of liability will remain effective, unless revoked in writing by the undersigned, and delivered to the aforesaid agent.
